Job summary

Northeast Georgia Health System is seeking a Registered Nurse to provide comprehensive assessment, planning, implementation and evaluation of patient care across age groups on the assigned unit.

You will collaborate with physicians, families and the care team, manage length of stay and discharge planning, participate in rounds, and ensure regulatory and safety standards are met; weekend rotation is required.

Qualifications

• Licensed RN in Georgia.
• Associate degree in Nursing from an accredited program.
• 3–5 years of direct patient care and/or case management experience.
• CCM or ACM certification preferred.
• Bachelor's degree preferred.

Responsibilities

• Monitor patients on assigned units to manage length of stay and resource use.
• Communicate with physicians, patients, families and the care team about discharge planning.
• Coordinate and facilitate the discharge plan for post-acute needs.
• Collaborate with Utilization Review to ensure appropriate level of care.
• Lead or participate in interdisciplinary rounds and ensure regulatory compliance.
