Job summary

Northeast Georgia Health System is seeking a capable Case Management Support professional to assist the RN Case Manager and Social Worker with discharge planning, continuum placement, and data collection. This role interfaces with patients, families, insurers, and post-acute providers to ensure smooth transitions.

The ideal candidate has healthcare experience, an active LPN license (GA preferred) or related health services degree, and strong communication and computer skills.

Qualifications

• Educational Requirements: High School Diploma or GED.
• Minimum Experience: Two years of healthcare experience.
• Current Georgia LPN license preferred or an Associate’s in Health/Human Services.
• Strong verbal, written, and interpersonal skills.
• Computer knowledge and data collection abilities.
• Ability to think creatively and act with urgency while managing resources.

Responsibilities

• Supports collaborative practice to coordinate patient care.
• Documents activities in the patient record with progress toward goals.
• Responds to referrals on the same day with timely record updates.
• Assists in monitoring length of stay and discharge planning.
• Facilitates post-acute placement and communicates with care providers and payors.
• Coordinates discharge services including transportation and follow-up.
• Works with Case Manager, Social Worker, nursing, and physician teams.
