Job summary

The Nurse Case Manager coordinates patient care across the healthcare continuum in Las Vegas. Primary focus includes discharge planning, care coordination, utilization review, insurance/resource coordination, and patient/family support.

This full-time role requires graduating from an accredited nursing program, an active Nevada RN license, and a minimum of 3 years RN experience in acute care, plus at least 1 year in case management, discharge planning, or utilization review.

Qualifications

• Graduate from an accredited nursing program.
• Minimum 3 years RN experience in acute care hospital.
• Minimum 1 year in Case Management, Discharge Planning or Utilization Review.
• Active Nevada RN license and BLS (AHA) preferred.

Responsibilities

• Discharge planning.
• Care coordination across the continuum of care.
• Utilization review and coordination of insurance/resources.
• Patient and family support and education.
• Multidisciplinary collaboration and acting as a clinical resource to patients, families, physicians, and nursing staff.
