Prime Healthcare Services at Dallas Regional Medical Center– Mesquite, TX seeks an RN Case Manager to oversee patient care from admission through discharge, coordinating with physicians, nurses, social workers, and other providers to ensure quality outcomes.

The role requires a current RN license, BLS, and experience in case management, discharge planning, or nursing management. Full-time, day shifts, with comprehensive benefits and ongoing professional development.

Formación

• Current and valid state RN License.
• BLS certification within 30 days and maintain current.
• Five years acute care nursing experience; at least one year in case management or discharge planning.
• Knowledge of Milliman Criteria and InterQual Criteria preferred.
• Basic to intermediate computer skills.
• Behavioral Violence Prevention Training within 3 months of hires and maintain current.

Responsabilidades

• Patient Assessment: evaluate patients’ medical, psychosocial, and discharge needs.
• Care Coordination: develop and implement individualized care plans in collaboration with the healthcare team.
• Discharge Planning: coordinate safe and appropriate discharge plans, including referrals to rehab or home health.
• Utilization Review: monitor hospital resources and services to ensure appropriate care.
• Patient and Family Education: help patients and families understand care plans and post-discharge instructions.
• Advocacy: advocate for patient needs and preferences, ensuring care aligns with goals.
