Job summary

Medical City Lewisville is seeking a Registered Nurse Case Manager for a full-time day shift (Thursday–Monday, 8:00am–4:30pm). You will coordinate the plan of care, manage length of stay, and ensure safe discharge planning in collaboration with the multidisciplinary team.

You will assess psychosocial and medical discharge needs, collaborate with Social Services, and guide patients toward post-acute resources, using InterQual criteria to support care decisions.

Qualifications

• Registered Nurse with current TX state license required.
• Associate Degree in Nursing required; Bachelor’s Degree in Nursing preferred.
• 3+ years of clinical hospital nursing experience required OR 2+ years experience in case management required.
• Certification in case management preferred.
• InterQual experience preferred.

Responsibilities

• Performs comprehensive assessment of psychosocial, medical and discharge needs of patients/family.
• Reassesses patient’s clinical condition and readmission risk; educates on accessing resources.
