Job summary

Sanford Health in Fargo, ND seeks an experienced registered nurse to lead care coordination and discharge planning across the patient care continuum. You will collaborate with licensed practitioners and inter-professional teams to optimize outcomes, manage resources, and ensure appropriate levels of care during hospitalization and after discharge.

Qualifications

• Bachelor's degree in Nursing required in Fargo and Sioux Falls; strongly preferred in Bemidji, Wisconsin, Michigan and Rapid City.
• Current unencumbered RN license in the state of practice and/or multi-state licensure if NLC state.
• Obtain and maintain Basic Life Support (BLS) within 60 days of hire; CCM certification encouraged when eligible.
