Qualifications

• RN licensure required (state-specific) and BLS within 60 days of hire.
• Bachelor's degree in Nursing strongly preferred; Fargo and Sioux Falls require it.
• CCM certification encouraged when eligible.

Responsibilities

• Coordinate discharge planning and resource management for patient care across the continuum.
• Collaborate with licensed practitioners and inter‑professional team to plan and evaluate care.
• Provide discharge‑planning education and make referrals to post‑acute resources.
• Advocate for patients across care settings to ensure appropriate level of care and timely discharge.
