Job summary

Sanford Health’s Marsh Med Ctr Park Falls is seeking a full-time registered nurse to provide expert nursing care and care coordination across the patient continuum. You will collaborate with licensed practitioners and inter-professional teams to ensure effective discharge planning and high-quality outcomes.

You will promote evidence-based practices, support patient education, and advocate for patients throughout hospitalization and after discharge in a day-shift role with 40 weekly hours.

Qualifications

• Bachelor's degree in Nursing strongly preferred.
• Unencumbered RN license required where practice occurs.
• Minimum of two years’ clinical experience preferred.
• BLS certification within 60 days of hire; CCM certification encouraged when eligible.

Responsibilities

• Provide individualized nursing care using the nursing process with a focus on patient education.
• Collaborate with physicians and inter-professional teams to plan, implement and evaluate care.
• Coordinate discharge planning and referrals to post-acute resources as needed.
• Advocate for patients across the continuum of care to achieve desired outcomes.
