Job summary

Sanford Health in the United States is seeking a Registered Nurse in a leadership role to provide expert resource management for patient care delivery across the continuum.

You will collaborate with licensed practitioners and inter-professional teams to coordinate care, plan discharges, apply the nursing process, and advocate for patients through hospitalization and post-discharge care.

Qualifications

• Bachelor's degree in Nursing strongly preferred.
• Currently holds an unencumbered RN license where practice occurs; state licensure as applicable.
• BLS certification required within 60 days of hire; CCM certification encouraged when eligible.

Responsibilities

• Provide care coordination and discharge planning across care settings.
• Collaborate with licensed practitioners and inter-professional team members to plan, implement and evaluate care.
• Utilize the nursing process (assessment, diagnosis, outcomes/planning, implementation, evaluation) to deliver individualized nursing care.
• Advocate for patients and families; facilitate smooth transitions from hospital to post-acute care.
