Job summary

Sanford Health in Fargo, ND is seeking an experienced registered nurse to join the South Fargo primary care team. You will provide individualized nursing care using the nursing process and coordinate with the inter‑professional team.

The role requires a Bachelor’s degree in Nursing, a current RN license, BLS, and a commitment to evidence‑based practice. Schedule is Monday–Friday, day shift, with no weekends and holidays, in a community‑focused setting.

Qualifications

Bachelor's degree in Nursing required in Fargo and Sioux Falls; other markets may differ.
Unencumbered RN license with state board of nursing; multi-state licensure (NLC) ok.
Graduate from an accredited nursing program (CCNE/ACEN/NLN CNEA).
Minimum of two years’ clinical experience preferred.
Basic Life Support (BLS) is required within six months of employment.

Responsibilities

Develops and provides individualized patient education based on assessed needs.
Develops and implements individualized care plans.
Assists with coordination of patient care across the continuum by collaborating with the inter‑professional team and community partners.
Works with the clinical team and may manage disease registries by identifying and following up with patients not meeting goals.
Provides services that focus on health and wellness.
Provides healthcare leadership, coordination of services, and implementation of evidence‑based practice.
Applies population health care management strategies to panels of patients.
Demonstrates competency within the full scope of nursing practice.
Functions within the Nurse Practice Act and state licensure requirements.
