Qualifications

Responsibilities

• Integrates health care for clients from pre‑admission to post discharge and coordinates care.
• Evaluate outcomes through integration of clinical, financial and utilization data.
• Communicate with patients, families and health care team to coordinate care and ensure smooth patient flow.
