Job summary

Sanford Health in Park Falls, WI is seeking a registered nurse with expertise in care coordination to lead patient care across the continuum and support discharge planning.

You will collaborate with physicians and inter-professional teams to optimize outcomes, advocate for patients, and ensure timely transitions from hospital to home or post-acute care.

A BSN and current RN license are required; BLS within 60 days; CCM certification encouraged where eligible.

Qualifications

• Bachelor's degree in Nursing; required in some markets.
• Current, unencumbered RN license in state of practice or multistate license.

Responsibilities

• Provide nursing care coordination across the care continuum.
• Collaborate with physicians and inter-professional teams to plan, implement and evaluate care.
• Coordinate discharge planning and education; facilitate timely transitions post-discharge.
• Advocate for patients and families and ensure appropriate level of care and resources.
