Job summary

Inova Fairfax Hospital is seeking an Experienced Registered Nurse Case Manager 1 to join the Case Management Team in Falls Church, VA. This Full-Time role (Mon-Fri with rotating weekends) focuses on discharge planning, care coordination, and ensuring appropriate utilization of services.

The RN Case Manager 1 collaborates with patients, families, and the interdisciplinary healthcare team to facilitate timely, safe, and cost-effective care transitions across acute and post-acute settings.

Qualifications

• Active RN license with case management experience.
• Experience coordinating discharge planning and care continuity.
• Ability to liaison between patients, families and clinicians.

Responsibilities

• Provide discharge planning and continuity of care for assigned patients in acute and post-acute settings.
• Coordinate services and act as liaison between patients, families and interdisciplinary healthcare team.
• Determine medical necessity and authorize payer processes when indicated.
• Participate in clinical performance improvement activities.
• Collaborate with team to manage discharge planning needs and post-discharge care.
