Qualifications

• Active RN license in Virginia or applicable state.
• Experience in hospital-based case management and discharge planning.
• Strong communication and Interdisciplinary collaboration skills.

Responsibilities

• Provide discharge planning and continuity of care for patients in acute and post-acute settings.
• Coordinate services and act as liaison among patients, families and care team.
• Utilize utilization management to assess medical necessity and efficiency.
• Ensure timely precertification and payer authorization where indicated.
• Participate in clinical performance improvement activities.
