Job summary

Avem Health Partners is seeking a passionate Case Manager to coordinate admission and discharge planning for patients, families, and guardians in Oklahoma. The role emphasizes interdisciplinary collaboration, maintaining comprehensive records, and ensuring regulatory compliance with patient rights and privacy.

The position requires an RN license in Oklahoma, a minimum of three years in case management, and BLS certification.

Qualifications

• Graduate of an accredited school of nursing.
• Current RN license in Oklahoma.
• BLS certification (or obtained within 30 days of hire).
• Certified Case Manager (CCM) certification preferred.

Responsibilities

• Conducts and documents a comprehensive initial assessment within 24 hours of admission.
• Coordinate admission and discharge planning with interdisciplinary team.
• Lead and participate in weekly interdisciplinary team meetings.
• Maintain progress notes and ensure appropriate referrals and social services are engaged.
• Coordinate with payor sources to assure proper reimbursement and appropriate care levels.
• Ensure HIPAA compliance and patient rights protections.

Education

Registered Nurse (RN) license

BSN preferred
