Saint Francis Hospital Muskogee, Inc. is seeking a Registered Nurse to join our Case Management team as a Case Manager. This role emphasizes discharge planning and coordination with the interdisciplinary team to ensure safe transitions of care for patients.

Oklahoma RN license is required; CCM certification is preferred. A minimum of 3 years of clinical RN experience is expected, with ECB status for department hours and limited benefit offerings.

Responsibilities

• Plans patient care with physicians and healthcare team.
• Ensures care and discharge plans align with patient’s clinical course.
• Identifies potential discharge barriers.
• Participates in multidisciplinary patient rounds.
• Coordinates discharge planning with care team, patient, and caregivers.
• Evaluates progress toward clinical/functional goals.
• Initiates discharge coordination from acute care.
• Obtains payer authorization for post-acute services.
• Arranges care settings (home health, rehab, SNF).
• Assists in hospital-to-hospital transfers.
• Evaluates readmission reasons to improve discharge activities.
• Tracks avoidable days and reports trends to leadership.
