Responsibilities

• Identifies admission reasons and co-morbidities to reduce readmission risk.
• Assesses patient self-care ability and ensures support systems are in place.
• Leads interdisciplinary team meetings to set attainable goals.
• Maintains weekly progress notes and documents outcomes.
• Collaborates with physicians and payors to coordinate care and reimbursement.
