Job summary

Atrium Health is seeking an experienced Registered Nurse Case Manager in Winston-Salem, NC. The role requires a BSN, MSN preferred, and at least five years of clinical experience with an emphasis on case management.

Licensure in NC is required, with CM certification encouraged within two years. The Case Manager will assess discharge needs, develop a Plan of Care with the care team, coordinate with physicians and payors, and monitor outcomes to ensure safe, cost-effective patient care.

Qualifications

• Graduate from an accredited school of nursing.
• BSN required; MSN preferred.
• Minimum 5 years of relevant clinical experience.
• Current RN licensure in the State where practicing.
• Certification in case management highly recommended within 2 years of employment.

Responsibilities

• Identifies patients who would benefit from Case Management interventions based on discharge needs.
• Assesses data by interviewing patient/family and evaluating patient needs.
• Develops and implements a Plan of Care with the team to improve outcomes and manage costs.
• Coordinates discharge planning with physicians, nurses, and community providers.
• Monitors effectiveness of discharge plans and adjusts as needed with the multi-disciplinary team.
• Documents actions in the EMR and communicates with providers and payors.
• Participates in rounds to align discharge planning with patient progression and institutional goals.
