Job summary

Advocate Health's Wake Forest Baptist Medical Center, Nursing: Care Coordination, is seeking a part-time Case Manager to identify discharge needs and coordinate multidisciplinary care. The role demands a BSN with at least 5 years of clinical experience and RN licensure in the state.

MSN is preferred. The position is PRN—Mon-Fri, 8am–5pm—requiring at least one shift per week, with eligibility for up to 19 hours weekly.

Qualifications

• Graduate from an accredited school of nursing.
• BSN required; MSN preferred.
• Minimum 5 years of relevant clinical experience.

Responsibilities

• Identify patients who would benefit from Case Management interventions.
• Assess data and information to plan care with the multidisciplinary team.
• Develop and implement a Plan of Care to optimize outcomes.
• Monitor and adjust Plan of Care as needed; document actions in EMR.
• Participate in multidisciplinary rounds and coordinate discharge planning.
• Ensure medical necessity and payer communication during transitions of care.
