Job summary

HonorHealth’s Deer Valley campus in Phoenix is seeking an RN Case Manager for the Case Management department. The day shift role (Mon-Fri, 8:00 am–4:30 pm) focuses on care coordination for high-risk acute care patients and discharge planning.

Qualifications include an Associate degree in Nursing, an active RN license (state licensure or compact), and at least 3 years of hospital clinical experience. Preferred: 5+ years in case management and CCM certification.

Qualifications

• 3 years of clinical experience in a hospital setting.
• Active RN license (State Licensure or Compact).
• Preferred: 5+ years in Case Management; 7 years RN experience; CCM preferred.

Responsibilities

• Guides care coordination for high risk acute care patients and facilitates discharge planning.
• Analyzes medical records to determine need for admission, treatment, and length of stay.
• Advocates for patients/families and liaises with the care team to ensure needs are met.
• Coordinates resources to ensure timely services and safe discharge.
• Collaborates with physicians and care teams for transitions of care.
