Job summary

HonorHealth in Phoenix, AZ is seeking a licensed RN Case Manager to coordinate care for high-risk acute patients and facilitate safe discharges. This day shift role includes a Monday–Friday schedule with one weekend per month rotation and a up to $10,000 sign-on bonus.

The ideal candidate holds an RN license in AZ (or state compact), a nursing degree (Bachelor's preferred; AD Nursing or diploma required), and 3 years hospital experience with 5–7 years in case management preferred.

Qualifications

• Bachelor's degree preferred for the RN Case Manager role.
• Associate's degree or diploma in Nursing required.
• 3 years of clinical hospital experience required.
• 5 years RN experience in Case Management preferred.
• 7 years RN experience preferred.
• Current AZ RN license or State Compact license required.
• CCM certification preferred.

Responsibilities

• Guides care coordination for high risk acute care patients and discharge planning.
• Analyzes medical records to determine admission legitimacy and length of stay.
• Advocates for patients/families and liaises with the care team to address needs.
• Collaborates with physicians to coordinate services and resources for timely care.
• Coordinates transitions of care to ensure safe discharge.
