Job summary

HonorHealth is recruiting a Registered Nurse Case Manager in Phoenix, AZ to guide care coordination for high-risk patients and manage discharge planning. The role requires analyzing medical records, advocating for patients, and coordinating with physicians and care teams to ensure timely services.

The position supports a day shift schedule, with one weekend per month rotation, located at John C Lincoln and surrounding facilities in the Phoenix area.

Qualifications

• Bachelor's Degree in Nursing or healthcare related field; BSN preferred.
• Current AZ RN license required.
• 3 years hospital clinical experience required.
• 5 years RN in Case Management preferred.
• 7 years RN overall preferred.

Responsibilities

• Guides care coordination for high risk acute care patients and facilitates discharge planning.
• Analyzes medical records to determine admission necessity, treatment, and length of stay.
• Advocates for patients/families and liaises with care team to ensure timely services.
• Coordinates with physicians to facilitate healthcare resources for safe discharge.
• Collaborates with patients, providers, and stakeholders to ensure appropriate care transitions.
