Qualificações

• Associates in Nursing required.
• Bachelor's in Nursing or related field preferred.
• 3 years hospital clinical experience required.
• 5 years RN Case Management preferred.
• 7 years RN experience preferred.
• Licensed Registered Nurse required.
• Certified Case Manager preferred.

Responsabilidades

• Guides care coordination for high-risk acute care patients and facilitates discharge planning.
• Analyzes medical records to determine admission legitimacy, treatment, and length of stay.
• Advocates for patients and liaises with the care team to address healthcare needs.
• Collaborates with physicians to facilitate timely healthcare resources and services.
• Coordinates services and referrals to ensure safe discharge.
