Job summary

HonorHealth is seeking an RN Case Manager to guide care coordination for high-risk acute care patients at Shea Medical Center in Scottsdale, AZ. You will oversee discharge planning for complex post-acute needs, analyze records, and advocate for patients while coordinating with physicians and the care team.

Ideal candidates have an RN license, 3+ years hospital experience, and experience in case management; outpatient exposure is a plus. Competitive benefits and a $10,000 sign-on bonus available.

Qualifications

• Associates in Nursing required.
• Bachelor's in Nursing or healthcare related field preferred.
• 3 years clinical experience in a hospital setting required.
• 5 years as an RN in Case Management preferred.
• 7 years as a Registered Nurse (RN) preferred.
• Experience in outpatient clinical settings with PCP, Home Health or Rehab preferred.
• RN license required; Certified Case Manager preferred.

Responsibilities

• Guides care coordination for high risk acute care patients and facilitates discharge planning for medically complex post-acute needs.
• Analyzes medical records to determine admission legitimacy, treatment and length of stay.
• Advocates for patients/families and liaises with the care team to address needs.
• Collaborates with physicians and coordinates resources to ensure timely services and safe discharge.
• Coordinates transitions of care and works with patients, providers and stakeholders.
