Job summary

Vibra Permanent Staffing is seeking an experienced and dedicated Registered Nurse Case Manager for a direct hire, full-time opportunity with one of our LTAC partners. This is your chance to join a specialized team that provides life-changing care to high-acuity patients recovering from complex medical conditions.

You will collaborate with physicians and an interdisciplinary team to coordinate individualized care plans, monitor patient progress, educate families, and advocate for long-term health

Qualifications

• Active RN license in the applicable state or compact state.
• 1+ year of case management experience preferred.
• BLS required; ACLS within 90 days of hire.
• Strong clinical judgment and commitment to quality patient care.
• Compassionate, team‑oriented mindset.

Responsibilities

• Provide direct care to critically ill or medically complex patients.
• Collaborate with physicians and an interdisciplinary team to coordinate individualized care plans.
• Monitor patient progress, administer medications and treatments, and document clinical findings.
• Educate patients and families, and advocate for long-term health and recovery.
• Maintain compliance with all policies, standards, and regulatory guidelines.
