Job summary

Vibra Permanent Staffing seeks an experienced Registered Nurse Case Manager (CM) for a direct-hire, full-time opportunity with LTAC partners. You will coordinate and oversee care for high-acuity patients, collaborating with physicians and an interdisciplinary team to implement individualized care plans.

The ideal candidate holds an active RN license in the state or a compact state, with 1+ year of case management experience preferred. BLS is required; ACLS within 90 days of hire.

Qualifications

• Active RN license in position state, or compact state.
• 1+ year of Case Management experience strongly preferred.
• BLS required; ACLS required within 90 days of hire.
• Strong clinical judgment and a commitment to excellence in patient care.

Responsibilities

• Provide direct care to critically ill or medically complex patients
• Collaborate with physicians and an interdisciplinary team to coordinate individualized care plans
• Monitor patient progress, administer medications and treatments, and document clinical findings
• Educate patients and families, and serve as an advocate for long-term health and recovery
• Maintain compliance with all policies, standards, and regulatory guidelines
