Job summary

Vanderbilt Health’s VCH Case Management team seeks a Case Manager to coordinate services across the continuum of care, balancing quality patient care with cost-effective resource utilization. The role is PRN, days (8a-4:30p), with a minimum commitment of 4 shifts in a 6-week cycle.

Responsibilities include coordinating admission, treatment, and discharge plans, ensuring care at the appropriate level, and liaising to secure reimbursement for hospital stays and services.

Qualifications

• Requires LIC-Registered Nurse licensure.
• Minimum 3 years of relevant work experience.
• Graduate of an approved discipline-specific program.

Responsibilities

• Coordinates admission, treatment, and discharge plans and ensures proper utilization of hospital services.
• Ensure care is provided in the most appropriate level for meeting patient care needs.
• Serves as a liaison to ensure appropriate reimbursement for hospital stays and services.
