Qualifications

• RN licensure (state of TN)

Responsibilities

• Assesses and identifies patients with complex clinical needs requiring a coordinated plan of care.
• Develops case management plans to attain optimal patient and family outcomes.
• Analyzes progress on the plan of care and removes barriers using process improvement methods.
• Advocates for patients, families and caregivers to ensure safe healthcare.
• Provides leadership and education to the team on complex needs and coordinating services.
