Job summary

Elwyn New Jersey in Vineland seeks a Nurse Case Manager to coordinate and implement health care services for individuals in community living programs and enhanced behavior homes.

The RN must be licensed in New Jersey with at least one year in general nursing or developmental disabilities, plus strong planning, communication, and documentation skills. CPR certification and EMR experience are required.

Qualifications

• Graduate of an accredited nursing program.
• Licensed Registered Nurse (RN) in New Jersey.
• One year general nursing or medical-surgical experience.
• One year of Developmental Disabilities nursing experience.
• Excellent planning, communication, customer interaction, collaboration, and documentation skills.
• Current CPR certification.
• Ability to work in a fast-paced environment with multiple priorities.
• Experience with EHR/EMR and MS Office applications.

Responsibilities

• Review ongoing medical, dental, and psychiatric treatment with the team.
• Provide site staff with training on health conditions and interventions.
• Review medical data, lab results, and diagnostic tests with the Treatment Team.
• Ensure timely follow-up with physicians and providers.
• Monitor weights and vital signs; report abnormalities to providers.
• Summarize health care follow-up in nursing progress notes.
• Maintain immunization status per NJ guidelines.
• Coordinate clinical services between CLP/EBSP and external providers.
• Oversee hospital discharge planning and continuity of care.
• Provide nursing on-call coverage during evenings/weekends.
