Job summary

The Jewish Board of Family and Children's Services seeks an RN Case Manager to bridge medical services with ICF/IRA residents, supervise LPNs and DSPs, and ensure nursing care within OPWDD and agency guidelines.

Responsibilities include ongoing assessments, care planning, care coordination, and staff training in medical procedures, medication administration, and diabetes care as part of a multidisciplinary team.

Qualifications

• Active, unrestricted New York State license as a Registered Professional Nurse.
• Bachelor’s degree and a minimum of 3 years clinical experience in a hospital or primary care setting preferred.

Responsibilities

• Provide direct nursing care as prescribed by physician/Nurse Practitioner in compliance with nursing standards and regulations.
• Perform nursing assessments upon entry and quarterly thereafter.
• Develop and implement an individualized nursing care plan and update annually or with clinical changes.
• Coordinate all health services and monitor treatment efficacy with clinical staff.
• Train staff in nursing tasks, medication administration, tube feeding and diabetic care.
• Maintain accurate, timely nursing notes and digital health records.
