HCA Florida JFK Hospital in Atlantis, FL is seeking a Registered Nurse Case Manager (PRN) to coordinate patient care and optimize transitions across the continuum. You will partner with physicians, nurses, and case management leadership to develop individualized plans reflecting each patient’s recovery goals.

The role emphasizes collaboration, documentation, and adherence to care standards while supporting responsive patient and family education and engagement.

Competenze

• RN with current state licensure; BSN preferred.
• Three years’ clinical, hospital nursing experience required; case management or critical care experience preferred.
• Certification in case management or utilization review preferred.
• InterQual experience preferred.

Mansioni

• Coordinate services across the care continuum to support safe transitions.
• Develop individualized care plans reflecting patient needs and recovery goals.
• Educate patients and families about treatment plans and discharge criteria.
• Document psychosocial assessments and communicate needs to the care team.
• Collaborate with Rehab Program Director and interdisciplinary team to ensure patient-centered care.
