Job summary

HCA Florida Fawcett Hospital is seeking a PRN Case Manager RN to support patients and families through care journeys, coordinating services, developing care plans, and ensuring safe transitions. You will partner with physicians, nurses, and leaders to maintain communication and high-quality care across the continuum.

The role emphasizes discharge planning, patient education, and collaboration with the Rehab Program Director and care teams.

Qualifications

• Associate Degree in Nursing or Nursing Diploma required.
• Bachelor’s degree in nursing preferred.
• 2+ year experience in case management OR 3+ years’ experience in clinical nursing required.
• Currently licensed as a Registered Nurse in the state(s) of practice according to law and regulation required.
• InterQual experience preferred
• Certification in Case Management preferred
• Ability to establish and maintain collaborative and effective working relationships
• Ability to communicate effectively in oral, written and electronic formats

Responsibilities

• Guide patients and families through program orientation and discharge planning.
• Coordinate education for patients and families to support treatment planning.
• Develop individualized treatment plans reflecting patient strengths and goals.
• Complete psychosocial assessments and document findings clearly.
• Collaborate with Rehab Program Director and interdisciplinary team for seamless care.
• Promote HCA Healthcare values in every interaction.
