Job summary

The Office of MaineCare Services, Complex Case Unit in Augusta, ME is seeking an experienced RN Case Manager to coordinate complex healthcare needs across MaineCare and community partners. The role focuses on case management, care coordination, discharge planning, utilization review, and collaboration with multidisciplinary teams to overcome barriers and achieve safe outcomes.

You will work hybrid in Augusta, reporting to the Complex Case Manager, and support both pediatric and adult populations

Qualifications

• Current, active RN license in Maine.
• Three (3) years of progressively responsible nursing experience.
• 2-3 years of clinical experience with pediatric and adult populations or prior case management experience.

Responsibilities

• Case Management & Care Coordination for individuals with complex health needs.
• Lead multidisciplinary team meetings and develop action plans.
• Coordinate with statewide resources and community organizations.
• Document, analyze, and track complex case information.
