Job summary

Mass General Brigham Community Physicians, Inc. is seeking a Complex Care Nurse (RN) Care Manager for a per diem, hybrid role in Newton, MA. You will coordinate care, manage referrals, and support patients and families across the continuum of care.

The successful candidate will hold an RN license in MA and NH, have at least two years in case management or discharge planning, and be proficient in coordinating with providers, payers, and community resources to ensure effective care transitions.

Qualifications

• Associate's Degree Nursing required; Bachelor's preferred.
• RN license for MA and NH; NH license transferable or willing to obtain.
• At least 2 years in case management, utilization review, or discharge planning.
• Ability to coordinate care with providers, payers, and families.

Responsibilities

• Client assessments and planning.
• Modify treatment plans based on patient responses.
• Prepare patient/family for discharge.
• Coordinate with third-party payers for authorizations.
• Review challenging cases with medical directors.
• Define care goals and educate patients and families.
• Maintain accurate reports and records.
