Job summary

Mass General Brigham Community Physicians, Inc. is seeking an experienced RN Case Manager to coordinate patient care within the facility and manage referrals outside the clinic. The role includes outreach and enrollment services and developing community care plans with families and providers.

Hybrid schedule with ~2 field days and ~3 days remote. MA RN licensure and 2+ years’ case management required; BSN preferred. Base pay reflects a broad range and competitive benefits.

Qualifications

• Associate's Degree Nursing required (BSN preferred).
• Active and unrestricted RN licensure for Massachusetts required.
• 2+ years in case management or discharge planning.
• Experience with complex dually eligible populations.
• Knowledge of Managed Care preferred.
• Understanding of diagnostic criteria for dual conditions and placement within continuum of care preferred.

Responsibilities

• Conduct client assessments and planning.
• Modify patient treatment plans as indicated by patients' responses and conditions.
• Prepare patient/family for discharge.
• Communicate with third party payers to obtain necessary authorization for reimbursement of services.
• Define care goals by providing education and direction to patients and families.
• Review cases with medical directors on challenging cases as needed.
• Maintain accurate, detailed reports and records.
