Office of MaineCare Services (OMS) — Complex Case Unit seeks an RN Case Manager to coordinate care for residents with complex needs, partnering with providers and state agencies to navigate barriers and achieve safe outcomes.

The role emphasizes multidisciplinary collaboration, discharge planning, utilization review, and robust documentation to support decision-making and patient advocacy.

Qualifications

• Current, active RN license in the State of Maine.
• Three (3) years of progressively responsible nursing experience.
• Minimum of 2-3 years of clinical work with pediatric and adult populations or prior case management.

Responsibilities

• Serve as resource and advocate for Maine residents and families facing complex care barriers.
• Assess, coordinate, and monitor services for individuals with chronic, acute, or medically complex needs.
• Lead multidisciplinary team meetings and develop action plans for case resolution.
• Collaborate with statewide resources, providers, and state agencies to address complex needs.
