Job summary

MaineHealth seeks a Case Manager I at Maine Medical Center to support the interdisciplinary team in coordinating utilization management, care facilitation and discharge planning. The role focuses on improving clinical outcomes, patient satisfaction and cost of care through effective collaboration with payors and care teams.

Qualified candidates have a BSN (or are in a BSN program within 1 year of hire) and an RN license in Maine, with 1–3 years of clinical experience.

Qualifications

• Education: BSN or must matriculate into a BSN program within one year of hire and completion within 5 years.
• License/Certifications: Current Maine RN license; Case Manager certification preferred.
• Experience: 1–3 years clinical practice; 3–5 years preferred.
• Excellent interpersonal communication and negotiation skills.
• Strong analytical, data management and PC skills.
• Knowledge of discharge planning, utilization management, case management, performance improvement and managed care reimbursement.

Responsibilities

• Facilitate collaborative management of patient care across the continuum to address barriers to timely and efficient care delivery.
• Apply process improvement methodologies to evaluate outcomes of care.
• Support and coaching of clinical documentation efforts.
• Coordinate communication with the interdisciplinary care team and payors.
