Duly Health and Care is seeking an RN Case Manager for the Care Ally program in Lombard, IL. This full-time role supports patients with complex medical needs and works with physicians and families to coordinate care across settings.

You will analyze data, enroll cases, and develop management plans while adhering to health plan requirements. The role emphasizes advocacy, collaboration, and resource coordination to improve outcomes.

Qualifications

• Must be an active Registered Nurse License in the State of Illinois in good standing.
• Certification in Case Management is preferred.
• Current CPR certification must be maintained (BLS).
• Associate's degree in nursing is required; Bachelor's degree preferred.

Responsibilities

• Promote quality outcomes for patients with complex medical needs.
• Manage patients across care settings to prevent inpatient readmissions.
• Coordinate health care services with physicians and providers.
• Develop individualized case management plans with prioritized goals.
• Complete documentation and case closures per policies and procedures.
