Qualifications

• Active Registered Nurse license in Illinois.
• 2–4 years of experience in primary care or hospital settings.
• Case management experience preferred.

Responsibilities

• Promote quality outcomes for patients with complex medical needs.
• Identify high-utilization cases and initiate intensive case management.
• Coordinate care across physicians, patients, and families.
• Enroll and manage caseload and complete attestations.
• Develop individualized case management plans and document activity.
