Qualifications

• Active Registered Nurse license in the State of Illinois (preferred Case Management certification).
• Associate degree in nursing required; bachelor's degree preferred.

Responsibilities

• Collaborate with physicians, patients, and families to promote quality outcomes for patients with complex medical needs.
• Increase efficient utilization of health care services and identify high-risk cases for intensive case management.
• Coordinate care across settings to ensure appropriate level-of-care and prevent readmissions.
