Job summary

Duly Health and Care is seeking an RN Case Manager for the Care Ally program in Carol Stream, IL. You will collaborate with physicians, patients, and families to improve outcomes for patients with complex medical needs.

The role focuses on efficient utilization of services, managing complex cases, and coordinating care across settings. Active IL RN license, BLS/ CPR, and 2–4 years of relevant experience are required; a case management certification is a plus.

Qualifications

• Active Registered Nurse license in Illinois in good standing (required).
• 2–4 years of RN experience in primary care or hospital settings; case management experience preferred.
• Certification in case management is a plus; CPR/BLS certification maintained.

Responsibilities

• Collaborate with physicians, patients, and families to promote quality outcomes.
• Identify chronic or complex cases and initiate intensive case management as per guidelines.
• Coordinate health care services and document all case activities and outcomes.
• Enroll and manage caseloads, complete assessments, and attestations per health plan requirements.
