Heart to Heart Hospice is seeking an RN Case Manager to provide comprehensive, patient-centered home-based care in the Seattle area. You will assess needs, develop and adjust the plan of care, and coordinate with physicians and the IDG to ensure quality end-of-life services.

Ideal candidates have a nursing degree, an active RN license, reliable transportation, and at least two years of healthcare experience.

Qualificações

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State Registered Nurse License.
• Current Driver's License with reliable transportation and provide ongoing valid auto liability insurance.
• Two years of experience as an RN preferred in a healthcare/clinical setting.

Responsabilidades

• Initial and ongoing comprehensive assessments of the impact of illness on patient’s physical, psychosocial, and environmental needs and ADLs.
• Implement the individualized plan of care and revise the plan as necessary.
• Collaborates care amongst patient, caregiver, physicians and healthcare professionals.
• Attend Inter-Disciplinary Group (IDG) meeting and coordinate patient care planning with IDG.
• Perform on-call responsibilities and provide on-call service to patients/families as assigned.
• Timely submission of all documentation.
• Perform other duties as required.
• Ensure compliance with local, state and federal laws, Medicare regulations, and established policies.
