Job summary

Heart to Heart Hospice is seeking a dedicated RN Case Manager to lead comprehensive patient assessments and coordinate care across the interdisciplinary team. You will implement and adapt the care plan, ensuring timely documentation and regulatory compliance while supporting patients and families through the course of terminal illness.

Qualifications include a nursing degree (Diploma/ASN/BSN), a current RN license, a valid driver's license with reliable transportation, and 2 years of RN

Qualifications

• Diploma/Associate/Bachelor of Nursing from an accredited program.
• Current State RN license.
• Valid driver's license with reliable transportation and auto liability insurance.
• Two years of RN experience preferred in healthcare/clinical settings.

Responsibilities

• Conduct initial and ongoing comprehensive assessments of the impact of terminal disease on patient’s physical, psychosocial, and environmental needs and ADLs.
• Implement the individualized Plan of Care (POC) and revise as necessary.
• Collaborate with patient, caregiver, physicians and healthcare professionals.
• Attend IDG meetings and coordinate patient care planning and prioritization of needs.
• Perform on-call duties and provide on-call service as assigned.
• Ensure timely documentation submission.
• Comply with local, state and federal laws, Medicare regulations, and internal policies.
