Job summary

Qualifications

• RN Compact license required
• Minimum 5+ years clinical practical experience with Medicare members
• Minimum 2+ years discharge planning and/or home health care coordination experience
• Associates degree or Nursing Diploma required
• Bachelors degree preferred

Responsibilities

• Facilitate the delivery of appropriate benefits and/or healthcare information
• Develops, implements and supports Health Strategies, tactics, policies and programs
• Case Manager uses collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services
