Job summary

Medical City Plano is seeking an experienced RN Case Manager to coordinate patient care from admission through discharge, ensuring efficient care plans and appropriate resource use.

The role involves assessing psychosocial and medical needs, collaborating with the multidisciplinary team, and guiding safe transitions using InterQual criteria to support patient-centered care and throughput.

Qualifications

• Registered Nurse with active TX license.
• Associate degree in nursing or nursing diploma required; Bachelor's preferred.
• 3+ years hospital nursing or 2+ years case management experience required.
• Case management certification and InterQual experience preferred.

Responsibilities

• Perform comprehensive assessment of psychosocial, medical and discharge needs.
• Coordinate plan of care and drive discharge planning with multidisciplinary team.
• Ensure appropriate post-acute needs and level of care in collaboration with Social Services.
• Refer to social services when psychosocial risks are identified.
• Communicate effectively with physicians, patients, families and other staff.
• Document care coordination interventions and discharge recommendations.
