Qualifications

Responsibilities

• Performs comprehensive psychosocial, medical, and discharge needs assessments.
• Reassesses clinical condition and risks for readmission; educates on resource access.
• Drives discharge planning in collaboration with physicians and social services.
• Refers to third-party payers and case management programs as needed.
• Documents recommendations and care coordination activities for the team.
• Represents patient needs and aligns resources for safe discharge.
