Qualifications

• RN license in New Mexico; case management experience desirable.
• BSN to be obtained within 5 years of hire date; ADN accepted with utilization review or case management experience.
• National Case Management certification preferred.

Responsibilities

• Manage a designated caseload and coordinate discharge planning across the continuum of care.
• Collaborate with physicians, patients, families, payers, and post-acute providers to improve outcomes and reduce discharge barriers.
• Lead utilization management and status determination using approved clinical criteria.
• Coordinate care transitions and ensure timely, safe discharge from admission to post-acute settings.
• Maintain accurate, timely documentation and contribute to performance improvement initiatives.
