Job summary

Medical City Plano is seeking an experienced RN Case Manager to coordinate patient care, optimize throughput, and ensure safe discharge planning with a multidisciplinary team. The role emphasizes evidence-based practice, regulatory compliance, and compassionate patient advocacy in a dynamic hospital setting.

Ideal candidates hold TX RN licensure, ADN or nursing diploma, 2–3+ years of acute care or case management experience, and InterQual knowledge; Bachelor's degree preferred.

Qualifications

• Registered Nurse with TX license required.
• ADN or nursing diploma required; Bachelor's degree preferred.
• 3+ years of clinical hospital nursing experience or 2+ years case management experience.
• Case management certification preferred.
• InterQual experience preferred.

Responsibilities

• Performs comprehensive assessments of psychosocial, medical and discharge needs.
• Reassesses clinical condition and readmission risk; educates on healthcare resources.
• Coordinates plan of care and drives discharge planning with multidisciplinary team.
• Partners with Social Services to ensure appropriate post-acute care.
• Refers to third party payer and disease management programs as needed.
• Documents recommendations and care coordination activities for communication with the health care team.
