Job summary

Houston Methodist is seeking a Case Manager (RN) to plan case management, care transitions, and discharge planning for a targeted patient population. The CM works with physicians and the care team to ensure compassionate, efficient, quality care and desired outcomes. It also coordinates with social workers to assure medical necessity and appropriate reimbursement.

The role emphasizes collaboration, documentation, and utilization management to support transitions to home or other care settings.

Qualifications

• RN with Texas licensure or ability to obtain within 60 days.
• Three years of hospital nursing clinical experience.
• Case management experience preferred.

Responsibilities

• Collaborates with physicians and interprofessional team for discharge planning.
• Participates in care transitions and coordination of services.
• Ensures medical necessity and appropriate admission/continued stay.
• Communicates clinical information to payors to support reimbursement.
