Job summary

MultiCare Health System is seeking a Hospital Case Manager RN to coordinate cross-continuum care and improve patient outcomes. The role partners with physicians and care teams to ensure safe transitions and optimal patient experiences across the continuum.

The ideal candidate holds a BSN, has an active WA RN license, and brings 4+ years of case management plus 5+ years of clinical hospital/ambulatory experience; an MSN/PhD/DNP/ARNP is preferred. Internal candidates may be considered.

Qualifications

• Bachelor's degree in Nursing required.
• Current Registered Nurse license in WA or MLS.
• Four years hospital case management experience within the last several years.
• Five years clinical hospital/ambulatory experience.
• MSN/PhD/DNP/ARNP preferred.
• Leadership and emergency services experience a plus.
• Internal candidates may be considered in lieu of some qualifications.

Responsibilities

• Coordinate transitions of care across the continuum for patients and families.
• Advocate for patients and families with transitions to lower levels of care when appropriate.
• Partner with physicians/APPs in patient care activities and care transitions.
• Apply critical thinking in all interactions to improve patient outcomes.
• Be accountable for transitions of care across the continuum.
